Complete and Return Form W-9

The Plan as described in the Policyholder Information Statement was approved and as provided in the Plan, eligible members received cash compensation. This cash compensation is taxable to you.

We ask that you complete Form W-9 and return it to Louisiana Farm Bureau Mutual Insurance Company if you have not done so already. You may submit the form either electronically or by mail.
